Open Payment Data Base Notice
For informational purposes only, a link to the federal Centers for Medicare and Medicaid Services (CMS) Open Payments web page is provided here: https://openpaymentsdata.cms.gov/. The federal Physician Payments Sunshine Act requires that detailed information about payment and other payments of value worth over ten dollars ($10) from manufacturers of drugs, medical devices, and biologics to physicians and teaching hospitals be made available to the public.
Disability Form Completion
We ONLY provide temporary total disability to patients who have recently undergone surgery.
CA State Disability (EDD): If you will be filing a CA State disability claim, please visit www.edd.ca.gov/Disability to file your claim online and complete necessary forms electronically. There is a fee of $40 for the completion of each disability form. It is the patient’s responsibility to contact our office with their receipt number once the claim has been submitted. Please allow 7-10 business days for completion.
FMLA and Other Disability forms: We have partnered with MediCopy to complete disability/FMLA forms. MediCopy is fully HIPAA compliant and adheres to all state and federal regulations concerning form completion.

Forms are $45 for the first form and $15 for each additional form turned in at the same time. - After payment is received, disability/FMLA forms are completed and sent to the requestor within three (3) business days.
